Check box disappeared

Hi, I like this plugin but I have a problem with the check box, I set everything, the text of the check box is ok but the little square is gone.
I had a look on the code and I noticed that <input type=”checkbox” name=”ny”> is 0×0 instead of 12×12
Can you help me please?
