If you’re using your own theme, you should update them starting from the example code of the default theme. Yours probably works, but the code it’s simpler on using the new format.
About subscribers’ migration, you means you are moving them to another blog? If the blog is the same, no migration is needed, all plugin’s versions uses the same tables on the database.
